Terms of Services

Last Updated: September 26, 2026

1. Agreement to These Terms

These Terms of Service ("Terms") are a legal agreement between you and Straighten Up Chiropractic Corp ("we," "us," "our," or the "Practice") governing your use of and any related pages, forms, and communications (together, the "Site").

By using the Site, you accept these Terms. If you do not agree, do not use the Site.

Our https://go.str810upad.com/privacy-policy-page explains how we handle information collected through the Site and is incorporated into these Terms by reference.

2. The Site Is Not Medical Advice

The content on the Site — including descriptions of conditions, treatments, techniques, articles, quizzes, assessments, body maps, videos, and any other material — is provided for general informational purposes only. It is not medical advice, diagnosis, or treatment, and it is not a substitute for consultation with a qualified health care provider.

Do not rely on the Site to diagnose or treat any condition. Always seek the advice of a licensed physician, chiropractor, physical therapist, or other qualified provider with any questions about a medical condition. Never disregard professional medical advice or delay seeking it because of something you read on the Site.

3. No Provider-Patient Relationship

Using the Site, submitting a form, requesting an appointment, or exchanging messages with us does not create a provider-patient relationship. That relationship begins only when you are formally accepted as a patient of the Practice, complete our intake and consent paperwork, and are evaluated in person by one of our licensed providers.

4. Medical Emergencies

The Site is not monitored continuously and must never be used to report a medical emergency.

If you are experiencing a medical emergency — including severe or sudden pain, numbness or weakness, loss of bladder or bowel control, difficulty breathing, chest pain, head injury, or any other urgent symptom — call your local emergency number or go to the nearest emergency room immediately. Do not submit a form, send a text, or send an email.

5. Appointment Requests

Submitting a request through the Site is a request only. It is not a confirmed appointment.

• An appointment is scheduled only when we confirm it with you directly, by phone, text, or email.

• Times shown on a scheduling page may not reflect real-time availability and can change.

• We may decline, reschedule, or cancel any request, including where our schedule is full, where your needs fall outside our scope of practice, or where we are not accepting new patients.

• If we determine that your condition is better addressed by another type of provider, we may refer you elsewhere rather than schedule you.

You are responsible for the accuracy of the information you provide. Incorrect contact details may prevent us from reaching you.

6. Cancellations, No-Shows, and Fees

Our current cancellation, rescheduling, no-show, and fee policies are provided to you at or before your first visit and may be updated from time to time. Fees quoted on the Site are estimates and are not a guarantee of the amount you will owe. Your actual cost depends on your evaluation, the services provided, and your insurance coverage.

7. Insurance and Coverage

Any statement on the Site about insurance carriers, networks, or coverage is general information and may change without notice. We do not guarantee that your plan will cover any service, or the amount it will pay. Coverage, deductibles, copays, and out-of-pocket costs are determined by your insurer, not by us. You are responsible for verifying your own benefits.

8. Promotional Offers

Any special offer, discount, package, or new-patient promotion advertised on the Site:

• Applies only to the services described in the offer

• Is available only to those who meet the stated eligibility conditions

• Cannot be combined with other offers unless we say so

• Is valid only for the stated period and may be withdrawn or changed at any time

• May be limited, modified, or unavailable where restricted by law, by insurance plan rules, or by the terms of any federal or state health care program

Individuals covered by Medicare, Medicaid, or another government health care program may not be eligible for discounted or promotional pricing. Contact us to confirm what applies to you.

9. Results and Testimonials

Patient stories, reviews, testimonials, before-and-after descriptions, and outcome statements on the Site reflect the experience of specific individuals. They are not a promise, guarantee, or prediction of the results you will get.

Outcomes depend on your diagnosis, medical history, severity and duration of symptoms, adherence to a treatment plan, and other factors outside our control. Individual results vary.

10. Eligibility and Age Restriction

The Site is intended for use by adults 18 years or older located in the United States. By using this service, you represent and warrant that you are at least 18 years of age. If you are under 18 years old, you may not use or access our services or opt into our messaging program.

If you are scheduling on behalf of a minor, you confirm that you are that minor's parent or legal guardian and are authorized to provide information and consent on their behalf.

11. Acceptable Use

When using the Site, you agree not to:

• Submit false, misleading, or impersonated information

• Submit information about another person without their permission

• Use the Site for any unlawful purpose, or in violation of any applicable law or regulation

• Attempt to gain unauthorized access to the Site, its servers, or any connected system

• Introduce malware, viruses, or any harmful code

• Scrape, harvest, crawl, or use automated means to extract data from the Site

• Interfere with the operation of the Site or with anyone else's use of it

• Copy, reproduce, republish, or redistribute Site content except as permitted in Section 14

• Submit content that is harassing, defamatory, obscene, or infringing

We may suspend or block access to the Site for any violation of these Terms.

12. Text Messaging and Communications

By providing your contact information, you agree that we may contact you by phone, text message, and email about your inquiry, appointment, and care.

Message Types. Our messaging program includes service updates, appointment confirmations, appointment reminders, and — where you have separately agreed — marketing promotions.

Operational Intent. This program is designed to ensure optimal customer support and to respond to your service-related inquiries.

Consent. Consent to receive marketing messages is not a condition of receiving care. Message and data rates may apply. Message frequency varies.

Opt-Out. You can cancel the SMS service at any time. Just text "STOP" to +1 615-909-4991. After you send the SMS message "STOP" to us, we will send you an SMS message to confirm that you have been unsubscribed.

Help. If you are experiencing issues with the messaging program, you can reply with the keyword HELP for more assistance.

Carrier Liability. Carriers are not liable for delayed or undelivered messages.

Email, text, and web forms are not secure channels. Do not use them to send detailed medical history, diagnoses, test results, or other sensitive health information. See our Privacy Policy for more.

13. Related Policy

Your use of the messaging program is also governed by our Privacy Policy. View it here: https://go.str810upad.com/privacy-policy-page.

14. Intellectual Property

The Site and its contents — text, graphics, logos, images, page design, layout, video, and software — are owned by us or our licensors and are protected by copyright, trademark, and other laws.

You may view and print Site content for your own personal, non-commercial use. Any other use — including reproduction, modification, distribution, republication, or public display — requires our prior written permission. Our name, logo, and marks may not be used without our written consent.

15. Third-Party Sites and Services

The Site may link to or embed third-party services, including scheduling tools, payment processors, review platforms, mapping services, and social media. We do not control those services, do not endorse them by linking to them, and are not responsible for their content, accuracy, availability, or practices. Your use of a third-party service is governed by that provider's own terms and privacy policy.

16. Site Availability and Changes

We may modify, suspend, or discontinue any part of the Site at any time without notice. We do not guarantee that the Site will be available, uninterrupted, error-free, or secure, or that content on it is complete or current.

17. Disclaimer of Warranties

The Site is provided "as is" and "as available," without warranties of any kind, express or implied. To the fullest extent permitted by law, we disclaim all warranties, including implied warranties of merchantability, fitness for a particular purpose, title, non-infringement, and any warranty as to accuracy, reliability, or availability.

Nothing in this section limits any warranty or protection that cannot be excluded under applicable law, and nothing in these Terms limits our professional obligations to you as an actual patient of the Practice.

18. Limitation of Liability

To the fullest extent permitted by law, neither we nor our owners, providers, employees, or service providers will be liable for any indirect, incidental, special, consequential, exemplary, or punitive damages, or for lost profits, lost data, or business interruption, arising out of or relating to your use of the Site — even if we have been advised of the possibility of such damages.

Our total liability for any claim relating to the Site will not exceed one hundred U.S. dollars ($100.00).

This section applies to your use of the Site only. It does not limit our liability for professional services rendered to you as a patient, and it does not apply to any liability that cannot be limited or excluded under applicable law. Some jurisdictions do not allow certain limitations, so parts of this section may not apply to you.

19. Indemnification

You agree to defend, indemnify, and hold harmless the Practice and its owners, providers, employees, and agents from any claims, damages, losses, liabilities, and expenses (including reasonable attorneys' fees) arising out of your use of the Site, your violation of these Terms, or your violation of any law or third-party right.

20. Governing Law and Venue

These Terms are governed by the laws of the State of TN, without regard to its conflict of laws rules. Any dispute arising out of or relating to these Terms or the Site will be brought exclusively in the state or federal courts located in Davidson County, TN, and you consent to the jurisdiction of those courts.

21. Changes to These Terms

We may update these Terms at any time. When we do, we will revise the "Last Updated" date above. Changes take effect when posted. Your continued use of the Site after a change means you accept the updated Terms. Review these Terms periodically.

22. General

• Severability. If any provision of these Terms is found unenforceable, the rest remains in full effect.

• No waiver. Our failure to enforce any provision is not a waiver of our right to enforce it later.

• Entire agreement. These Terms and the Privacy Policy are the entire agreement between you and us regarding the Site.

• Assignment. You may not assign these Terms. We may assign them in connection with a merger, acquisition, or sale of the Practice.

• Survival. Sections 14, 17, 18, 19, and 20 survive any termination of your use of the Site.
