Privacy Policy

1. About This Policy

Straighten Up Chiropractic Corp ("we," "us," "our," or the "Practice") operates and related marketing pages, forms, and communications (together, the "Site"). This policy explains what information we collect through the Site, how we use it, and the choices you have.

This policy covers our website and marketing activities only. If you become a patient of Straighten Up Chiropractic, your medical records and treatment information are governed by a separate document — our Notice of Privacy Practices — which describes your rights under applicable health privacy laws. The two documents are not the same, and this policy does not replace it. Ask our front desk for a copy of the Notice of Privacy Practices, or request one at [email protected].

By using the Site, you agree to the practices described here. If you do not agree, please do not use the Site.

• The reason you are contacting us, including any details you choose to share about your symptoms, condition, or goals

Please do not send detailed medical history, diagnoses, test results, or other sensitive health information through website forms, text messages, or email. These channels are not secure. Share that information with us in person, over the phone, or through a secure patient portal if one is offered.

Information collected automatically

When you visit the Site, we and our technology providers may automatically collect:

• IP address and approximate geographic region

• Browser type, device type, and operating system

• Pages you viewed, links you clicked, and how long you stayed

• The website, ad, or search that referred you

• Date and time of your visit

Information from other sources

We may receive information about you from advertising platforms, appointment scheduling tools, review platforms, or referral partners, and may combine it with the information described above.

3. How We Use Your Information

We use the information we collect to:

• Respond to your inquiry and contact you about scheduling

• Confirm, remind you about, reschedule, or follow up on appointments

• Answer questions about our services, hours, location, or insurance

• Send you marketing communications about the Practice, where you have agreed to receive them

• Operate, maintain, and improve the Site

• Measure how well our advertising and marketing are working

• Detect and prevent fraud, abuse, and security incidents

• Comply with legal obligations and enforce our terms

4. Text Messaging and Phone Calls

If you provide your phone number and consent, we may contact you by phone call and SMS/MMS text message about appointment scheduling, reminders, follow-up, and — where you have separately agreed — promotional offers.

Purpose. Messages you receive from us are sent to deliver appointment confirmations, appointment reminders, and responses to your service inquiries.

Consent is not a condition of purchase or of receiving care. Message and data rates may apply. Message frequency varies.

Opt-out. You can cancel the SMS service at any time. Just text "STOP" to +1 615-909-4991. After you send the SMS message "STOP" to us, we will send you an SMS message to confirm that you have been unsubscribed.

Help. If you are experiencing issues with the messaging program, you can reply with the keyword HELP for more assistance, or contact us at +1 615-909-4991.

Data sharing. No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

Text messaging is not a secure channel and is not appropriate for urgent medical issues. If you are experiencing a medical emergency, call your local emergency number immediately.

5. Cookies, Tracking, and Advertising

The Site uses cookies, pixels, tags, and similar technologies. These help the Site function, remember your preferences, measure traffic, and evaluate the performance of our advertising.

We may work with advertising and analytics providers — which may include social media platforms and search engines — that use these technologies to understand how visitors arrive at and use the Site, and to show you ads for the Practice on other websites and apps.

Limits on what we share with advertising platforms. We configure our tracking so that information about specific symptoms, conditions, diagnoses, treatments, or appointment reasons is not transmitted to advertising or analytics platforms. Advertising events sent from the Site are limited to general activity such as "visited a page" or "submitted a form," without the content of what you submitted.

Your tracking choices

• Browser settings. Most browsers let you block or delete cookies. Blocking cookies may cause parts of the Site to stop working correctly.

• Do Not Track. The Site does not currently respond to browser Do Not Track signals, as there is no common industry standard for them.

• Ad settings. You can adjust ad personalization in the settings of the platforms you use, and through industry opt-out tools such as the

Digital Advertising Alliance and Network Advertising Initiative opt-out pages.

6. When We Share Information

We share personal information only as described below. We do not sell your personal information.

• Service providers. Companies that operate the Site and support our practice on our behalf — website hosting, form and scheduling

tools, customer relationship management and marketing automation platforms, email and text messaging providers, phone systems, analytics

providers, and our marketing agency. These providers may use your information only to perform services for us.

• Within the Practice. Our staff, providers, and administrative team, as needed to respond to you and coordinate care.

• Legal and safety. When required by law, subpoena, court order, or government request; to enforce our agreements; or to protect the

rights, property, or safety of the Practice, our patients, or the public.

• Business transfers. In connection with a merger, acquisition, financing, or sale of all or part of the Practice, subject to applicable law.

• With your direction. Any other sharing you ask us to do or agree to.

7. Your Choices and Rights

• Email. Click the unsubscribe link in any marketing email.

• Text. Reply STOP to any message.

• Phone. Ask us to remove you from our call list.

• Access, correction, deletion. You may ask us for a copy of the personal information we hold about you, ask us to correct it, or ask us to

delete it. Contact [email protected].

Depending on where you live, you may have additional rights under state or national privacy law, including rights to know what information we collect, to delete it, to correct it, to opt out of targeted advertising, and not to be discriminated against for exercising those rights. We will verify your identity before acting on a request and will respond within the time frame the applicable law requires.

Some information cannot be deleted on request — for example, patient records we are legally required to retain, or information we need to keep to comply with law, resolve disputes, or prevent fraud.

8. Data Retention

We keep information collected through the Site for as long as needed to respond to you, provide services, meet our legal and recordkeeping obligations, and resolve disputes. Retention periods differ depending on the type of information and the reason we hold it. Patient records are retained according to the requirements that apply to health care providers in our state.

9. Security

We use reasonable administrative, technical, and physical safeguards designed to protect information collected through the Site, including SSL encryption for form submissions. No website, internet transmission, or electronic storage method is completely secure, and we cannot guarantee absolute security. You send information to us at your own risk.

10. Age Restriction

By using this service, you represent and warrant that you are at least 18 years of age. If you are under 18 years old, you may not use or access our services or opt into our messaging program.

Your use of the messaging program is also governed by our Terms & Conditions. View it here: https://go.str810upad.com/terms-of-services.

14. Changes to This Policy

We may update this policy from time to time. When we do, we will revise the "Last Updated" date at the top of this page. Material changes may be announced more prominently on the Site. Continued use of the Site after a change means you accept the updated policy.
